The Royal Blue Maltese Cross is the symbol of the City of Amalfi Italy, (on the Sorrentine Peninsula) where the Knights of St. Johns Hospitalier was founded by Amalfitan merchants back in the 13th century.
The blue is still used by the St. Johns Hospitalier as their emblem.
